Location:	Located on a privately owned farm at 2444 North State Route 48. The farm is located about 1/4 mile south of Kirby Road. The cemetery is in a field about 500 feet east of Route 48.

Cemetery History	The tombstones are located on private property of the Triple Creek Family Farm, on a hill where cattle graze, east of the creek. Four tombstones were found. Two old stones for Revolutionary War veteran, John Hormell who died in 1823 and his wife Elenor who died in 1821. The other two stones are replacement stones for John Hormell and his wife Elenor, recently placed (within the last 3-5 years) by some descendants.
